Admission Requirements

Children from 2-4 years of age will be accepted provided the parents complete an On-line Enrollment Application, a Child In-take questionnaire and Request

for records (if applicable). In addition, a copy of the child’s birth certificate, social security card, a completed Record of Immunization Form-DH 680 (blue) and

Health Certificate Form-HD 3040 (yellow) must be submitted prior to admission.

VPK Program

Your child is eligible to participate in Florida's FREE (VPK) Voluntary Pre-kindergarten Education Program if you live in Florida, and your child turns 4 years of

age by September 1st. Parents must submit a VPK certificate in addition to the above mentioned forms prior to admission.
